CORE Program, formerly the Adult Outpatient Services Transformation

Sacramento County has transformed the Wellness and Recovery Center, Regional Support Team, Guest House, and TCORE Programs into the new Community Outreach Recovery Empowerment (CORE) Program. The CORE Program is operated by the following agencies: Bay Area Community Services (BACS), El Hogar, Hope Cooperative, and Turning Point.

CORE was designed through feedback and input gathered from consumers, family members of consumers, direct service staff as well as various community stakeholders. CORE combines what we heard is valued most and adds new enhanced services.

We'll Meet You There

CORE provides specialty mental health outpatient services at 11 sites throughout Sacramento County. In addition to these sites, services are delivered in the community, wherever requested. Each CORE site has a co-located peer run community wellness center, which is open to any Sacramento County adult seeking meaningful activities. Individuals can self-refer to a CORE program by reaching out to Sacramento Access Team (916-875-1055 or toll free 888-881-4881) for a screening or by walking into a CORE provider site during the designated walk-in hours for an assessment. Recovery Happens

Services are strength-based, recovery focused, and flexible. As your needs change, your level of treatment intensity can be adjusted.
