Outbreak Reporting Module
The outbreak reporting module is intended for the following facilities to report outbreak information to Sacramento County Public Health as required by Title 17:
- Childcares, Daycares, and Preschools
- Correctional Facilities
- Health Care Providers
- Homeless Shelters
- Schools
This module is not intended for reporting by the general public or non-outbreak related cases.
Please review this section before reporting:
- Information requested includes: facility information (point of contact, facility name and address, staff and case counts) and specific case information depending on the facility type (demographics, symptoms, testing, vaccination). Preview the questions: businesses, childcares/daycares/preschools, correctional facilities, health care providers, homeless shelters, schools. Note: some questions have been added or removed with the updated version as of 6/6/23.
- Submit responses within 24 hours after starting the reporting session. Responses not submitted within 24 hours will not be recorded.
Outbreak Reporting Resources:
Note: As of January 1, 2023, businesses in Sacramento County are no longer required to report COVID-19 outbreaks to Sacramento County Public Health. Please note that this does not apply to long-term care facilities and skilled nursing facilties. If you have any questions, please call (916) 875-5881.
