How To Apply
Qualified Sacramento County residents who wish to apply for the Medical Marijuana Identification Card (MMIC) program must provide the following documents:
- Completed MMIC Application/Renewal form - CDPH 9042. For applicants with caregivers, the caregiver section must also be completed.
- Proof of identify. This can be a valid California Department of Motor Vehicles (DMV) driver's license or identification (ID) card, or other valid government-issued photo card.
- Proof of residency. This can be a current and valid California DMV motor vehicle registration, current rental or mortgage agreement, or utility bill. This proof must have patient’s name bearing current physical address within the county.
- Written documentation from your physician or form CDPH 9044 (Written Documentation of Patient's Medical Records) recommending the use of medical marijuana as appropriate for one or more serious medical condition.
- Your physician’s name, contact information, and California medical license number.
- Non refundable application fee of $100.00 (or $50.00 for Medi-Cal beneficiaries; Medi-Cal card required).
Application packets are available at the Sacramento County Office of Vital Records, 7001-A East Parkway Suite 650, Monday through Friday, 8:00 am to 4:00 pm or by calling (916) 875-2066. Appointments are scheduled for Mondays and Fridays in the morning at 9:00 or 10:00 am and in the afternoon at 1:30 or 2:30 pm.
For primary caregivers
- The caregiver must be accompanied by the patient at the time of the appointment.
For minors
- Proof of emancipation; non emancipated minors must be accompanied by a parent or guardian at the time of the appointment.
- Parent(s) proof of residency must be presented. This can be a current and valid DMV motor vehicle registration, current rental or mortgage agreement, or utility bill. This proof must have the parent's name bearing current physical address within the county.
