Homeless Engagement and Response Team (HEART)

Sacramento County’s Homeless Engagement and Response Team (HEART) is a field and shelter based outreach team that supports individuals experiencing homelessness get linked to behavioral health services. The team, which consists of Mental Health Counselors and Behavioral Health Peer Specialists, engages individuals in the community connecting them to vital resources including mental health treatment, substance use treatment, medical care, shelter, and more.
Behavioral Health Bridge Housing

Behavioral Health Bridge Housing (BHBH) provides interim housing for individuals and families who are experiencing homelessness and living with serious mental illness or substance use disorder. This program connects housing with behavioral health treatment. Stable housing provides the foundation people need to focus on their health, while treatment helps individuals stabilize and maintain housing. Together, housing and services work hand-in-hand to support recovery and long-term stability. Stable housing helps people focus on their health. Treatment helps people stay housed. The two work together.
