Outbreak Reporting Module
The outbreak reporting module is intended for the following facilities to report outbreak information to Sacramento County Public Health as required by Title 17:
- Childcares, Daycares, and Preschools
- Correctional Facilities
- Health Care Providers
- Homeless Shelters
- Schools
This module is not intended for reporting by the general public or non-outbreak related cases.
For reporting individual (non-outbreak) cases, refer to the Reportable Diseases and Conditions page for more information.
Please review this section before reporting:
- Information requested includes: facility information (point of contact, facility name and address, staff and case counts) and specific case information depending on the facility type (demographics, symptoms, testing, vaccination).
- Submit responses within 24 hours after starting the reporting session. Responses not submitted within 24 hours will not be recorded.
