​What is CalAIM?

​​California Advancing and Innovating Medi-Cal (CalAIM) is a multi-year initiative by the Department of Health Care Services (DHCS) to improve the quality of life and health outcomes of our population by implementing broad delivery system, program and payment reform across the Medi-Cal program. The major components of CalAIM build upon the successful outcomes of various pilots (including but not limited to the Whole Person Care Pilots (WPC), Health Homes Program (HHP), and the Coordinated Care Initiative) from the previous federal waivers and will result in a better quality of life for Medi-Cal members as well as long-term cost savings/avoidance.


CalAIM Goals:

  1. ​Identify and manage member risk and need through whole person care approaches and addressing Social Determinants of Health;
  2. Move Medi-Cal to a more consistent and seamless system by reducing complexity and increasing flexibility; and
  3. Improve quality outcomes, reduce health disparities, and drive delivery system transformation and innovation through value-based initiatives, modernization of systems, and payment reform.

​​Enhanced Care Management and Community Supports

Enhanced Care Management (ECM) and Community Supports are foundational parts of the transformation of Medi-Cal focused on:

  • Breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities; 
  • Introducing a better way to coordinate care; and
  • Providing high-need members with in-person care management where they live.​


​Enhanced Care Management is a new statewide Medi-Cal benefit available to eligible members with complex needs, including:
  • ​Access to​​ a single Lead Care Manager who provides comprehensive care management and coordinates their health and health-related care and services
  • Connections to the quality care they need, no matter where members seek care--at the doctor, the dentist, with a social worker, or at a community center
  • Help with better understanding medications
  • Setting up rides to doctor's visits
  • Access to care after hospitalization
  • Connections to more services like food and housing​ 
​Community Supports are services that help address members' health-related social needs, help them live healthier lives, and avoid higher, costlier levels of care. These include:
  • Support to secure and maintain housing (coming soon!)
  • Personal care and home services, making a member's home safer and more livable to help members stay health at home or reduce asthma
  • Access to home-delivered meals to help with nutriion-sensitive health conditions and diet needs
  • A variety of other community-based services


​Community Support services are available to a wide range of members, including those with complex needs, and regardless of whether they qualify for ECM services. Below is the list of the 14 pre-approved Community Supports which all Managed Care Plans (MCPs) in Sacramento County offer:

  • ​Housing transition/navigation (coming soon!)
  • Housing deposits (coming soon!)
  • Housing tenancy & sustaining services (coming soon!)
  • Short-term post-hospitalization housing
  • Recuperative care (medical respite)
  • Respite services
  • Day habilitation programs
  • Nursing facility transition/diversion
  • Community transition services/nursing facility transition to a home
  • Personal care and homemaker services
  • Environmental accessibility adaptations (home modifications)
  • Medically tailored meals/medically supportive food
  • Asthma remediation
  • Sobering centers​
