Instructions for EMR Certification Reinstatement - 6 to 12 Month Lapse
Apply online at https://actonline.saccounty.net. Online application instructions. DO NOT create a new account if you have previously created one to do an application. If you have difficulties logging in, please call our office or the Sacramento County Help Desk (916) 874-5555 for assistance. To be reinstated as an EMR by Sacramento County, the applicant complete and upload the following: Expired EMR Certification issued in Sacramento County. Successful completion of a 12 hour refresher course from an approved EMR training program, and 12 hours of continuing education, within the 24 months prior to applying for renewal OR; 24 hours of continuing education, within the 24 months prior to applying for recertification/reinstatement, from an approved CE provider. CE courses are required to be no less than 1.0 hours (0.5 hour courses cannot be counted).
Skills competency verification form, with original signatures. Government issued photo identification (Dirver's License, Passport, Military ID). Current CPR for the Healthcare Provider card or equivalent.
Upload a “passport-style" color photo of yourself.
Supplemental Certification Application (if prior convictions) and all documents that pertain to prior conviction(s). Have your employment verified by your chosen employer (an email will be sent to your employer to which they need to respond in order to verify your current employment). Pay the required fees.
Once the above has been submitted, please allow 1-2 weeks for your application to be processed.
