Information Communicated: Hello Partners and Colleagues,

Thank you to everyone who attended the October 9, 2026, Housing Supports Plan (HSP) Policy and Procedure training. We appreciate your participation, thoughtful questions, and continued commitment to supporting the housing stability of the members we serve.

As a follow-up, we are sharing the training recording and additional resources to support implementation of the HSP requirements and assist providers in developing comprehensive, individualized housing plans.

We encourage you to review these materials and use them as references when developing and updating HSPs. The completed case examples are intended as training tools to demonstrate effective documentation and individualized housing planning. Our goal is to support a consistent, coordinated, and person-centered approach to housing planning that helps members obtain and maintain stable housing. Thank you again for your engagement and partnership in this work.

Please don't hesitate to reach out to myself, Erica Jaramillo, jaramilloe@saccounty.gov and/or Kandyce Seely, seelyk@saccounty.gov if you have questions or need additional clarification as you begin implementing the HSP requirements. Best regards,

Greetings,

Please see the attached memo regarding updated documentation requirements for members receiving BHSA Housing Intervention benefits who have a current or historical experience of chronic homelessness. The memo provides direction on the steps providers must take to ensure SmartCare documentation accurately supports BHSA
reporting requirements.

In summary, providers are asked to complete the following by August 12, 2026:

  • Review all current members who received BHSA Housing Intervention benefits
on or after July 1, 2026 (including members who have since been discharged).
  • Verify whether the member met the definition of chronic homelessness before, or at the time they began receiving BHSA Housing Intervention benefits.
  • Update the SmartCare Special Population designation using the earliest supportable date.
  • Ensure the member's Problem List includes the appropriate housing-related ICD-10-CM Z Code.
  • Verify that supporting documentation is present in the record.
  • Continue to keep Special Population designations and housing-related documentation current as members' housing situations change.

The attached memo includes detailed instructions, examples of acceptable documentation, SmartCare screenshots, and guidance for updating both the Special Population field and the Problem List.

Thank you for your partnership and for helping ensure our documentation accurately reflects the members we serve and supports Sacramento County's BHSA reporting requirements.

This email is being sent out on behalf of CQI, EHR and SUPT.
Greetings Residential Treatment Providers,
We understand that this update may be unexpected and may cause frustration, and we truly appreciate your patience and continued partnership as we work through these recent denials together.
It has come to our attention through provider denials that Residential Treatment Providers (including Withdrawal Management) are not permitted to use Z03.89 or any of the Z55–Z65 codes when claiming the daily rate.
To correct these denials, a diagnosis update must be completed using an approved F code from the list in the DMC-ODS Billing Manual , beginning on page 111. When selecting the replacement for Z03.89 or any of the Z55–Z65 codes, please review the assessment information documented in the ASAM and BQUIP for each member. Your LPHA should determine the most appropriate diagnosis from the approved list based on the information gathered—and only if clinically appropriate to the service provided.
Our CQI/QM team will be updating training materials and the DMC ODS DSM–ICD 10/ SNOMED Crosswalk to reflect these changes so that Residential Treatment Providers have clearer guidance moving forward.
To avoid future recoupments, timely review of denials is essential. Please remember that corrections must be completed within 15 months, and the EHR team requires approximately 4 weeks to process replacements. Please run and work on the Program Denials Report in order to work these denials. We encourage you to take prompt action to ensure compliance.
If you need assistance:
• For denial corrections, please contact BHS-EHRBilling@saccounty.gov
• For documentation-related questions, please contact QMInformation@saccounty.gov
