Overview
Sacramento County Full Service Partnerships (FSP) provide a variety of high intensity services to eligible families (youth and parents), Transition Age Youth (TAY) age 18 and up, adults, and older adults. These services provide 24/7 support to individuals struggling with persistent and significant mental illness, who would otherwise utilize the most restrictive and highest level of care, such as psychiatric hospitalization and incarceration.
The FSP program provides comprehensive, integrated, culturally competent, community-based mental health services, which include assessments, planning, 24/7 crisis response, individual and group treatment, social rehabilitation, case management, psychiatric medication services, and housing services and supports to address the needs of individuals living with serious mental illness who are homeless or at-risk of homelessness. FSP provides utilize a "whatever it takes" approach to assist with benefit acquisition, employment, education, transportation, and supportive services to family members/caregivers, such as education, consultation, collaboration with system partners and interventions to support members in maintaining stability and social connectedness in the community and working toward recovery.
How to access FSP Services:
Access to Adult and TAY FSP programs are through the Intensive Placement Team (IPT) and a Level of Intensity Screening Tool (LIST) to support eligibility and linkage. JJDTP is direct referrals from Sacramento youth probation.
