​​​Overview

The Intensive Placement Team (IPT) serves as an access point for referrals to the Full Service Partnership (FSP) programs, Augmented Board and Care program (ABC), Adult Residential Treatment program (ARTP), and sub-acute services. 

IPT is made up of a clinical team responsible for reviewing the Level of Intensity Screening Tool (LIST) and determining service authorizations for FSP's and sub-acute residential placement. The IPT program monitors and ensures that the scope of services for ABC and ARTP programs are provided to fidelity, for individuals who have been authorized and admitted by IPT. The program also monitors the care of individuals receiving treatment in contracted subacute locked settings to support clients in their recovery, with the goal of returning to the community.


How to Access IPT Services

​IPT services are accessed through referral only from a provider or hospital. 

Providers looking to refer a client to FSP services can complete the Level of Intensity Screening Tool (LIST).

Hospitals wanting to refer a patient for sub-acute services can do so by submitting a LIST request. The IPT team will then conduct a LOCUS (Level of Care Utilization System) assessment with the treatment team. Information for this evaluation will be gathered from key people in the client's support system, such as family and the client's LPS Conservator.


IPT Services:

Augmented Board and Care

Augmented Board and Care (ABC) homes are licensed residential board and care homes with extensive experience in working with individuals struggling with chronic mental illness. The goal of the ABC program is to keep the client in the community, to avoid psychiatric hospitalization as well as incarceration. ABC homes provide significant structure and in-home programming in a home-like environment. An individual admitted to an ABC home must also be admitted to an FSP.  Psychiatric supports and treatments are offered in partnership with our outpatient FSP programs to support our ABC clients journey to recovery. The ABC Team oversees the careful coordination between the ABC operator, the FSP, the conservator and the client's natural support system until the client is ready to step-down to a less intensive placement setting.

Eligibility Criteria: 

  • Must be enrolled in an FSP
  • ​Clients must have some complicating factor such as a history of medication management challenges, difficulty engaging with mental health providers, substance challenges, etc.​


​​Adult Residential Treatment Program

The Adult Residential Treatment Program (ARTP) offers placement in a semi-secured, delayed egress, structured, residential care facilities. These facilities provide on-site psychiatric supports as well as in-house treatment and programming. They strive to provide a home-like environment with a moderate level of structure as part of Sacramento County's sub-acute continuum. The IPT Team visits all ARTP sites and are evaluating each client on a monthly basis. LPS conservatorship is preferred by most ARTP providers. The goal is to return all clients to a lower level of care. 

Eligibility Criteria:

  • Must have a LOCUS recommendation of Level 5 and/or the hospital treatment team's, the FSP and IPT team's recommendation for a higher level of care.  
  • ​Client must be deemed likely to be successful in a more structured semi-secured or unsecured setting.
  • Be on LPS Conservatorship through the State of California. 


Subacute

Subacute facilities are fully secured, highly structured care facilities that provide onsite psychiatric supports, treatment, and intensive programming. They strive to provide a home-like environment with a high level of intensity and structure, as part of Sacramento County's sub-acute continuum. Clients must be on a LPS or Murphy's conservatorship. The IPT Team visits all subacute sites and evaluates each client on a monthly basis to help in their recovery. The goal is to return all clients to a lower level of care and eventually, back to the community. ​

Eligibility Criteria:

  • Must have a LOCUS recommendation of Level 5 and/or the hospital treatment team's, the FSP and IPT team's recommendation for a higher level of care.  
  • Client must be deemed likely to be successful in a more structured semi-secured or unsecured setting.
  • Be on LPS Conservatorship through the State of California. ​
