Healthy Brain Initiative


Description

​​​​​​​​​​​​​​​​​​​​​​​​The goal of the Sacramento County Healthy Brain Initiative Program is to increase access to and use of existing resources for those providing care for older adults with Alzheimer's disease and related dementias (ADRD). This program is part of the Older Adult Health Program along with the Fall Prevention​  (StopFalls​ Sacramento).  The program focuses on promoting and providing support to: 

  • Educate on brain health and cognitive aging using evidence-based programming including:
    • Dementia Dialogues which is a nationally registered, evidence-informed 5-module training program to educate caregivers and community members on Alzheimer’s disease and related dementias (ADRD). It covers dementia overview, communication strategies, environment, independence, challenging behaviors, and creative problem-solving. 
    • Virtual Dementia Tours is an educational and training tool for building empathy to improve the care provided to people living with dementia.
  • Increase access to community resources
  • Promote Healthy Aging

This program is partnered with various local organizations to effect change at an individual, community, and policy level. The program includes educational training, local resources, and guides, and collaborative efforts with community partners. 

Sacramento County Healthy Brain Initiative Program is currently funded by the California Department of Public Health (CDPH) Healthy Brain Initiative (HBI).  Funding is allocated through June 2025.
