Who We Are

Noel Vargas currently serves as the Deputy Director of Primary Health Services for the County of Sacramento, which includes Adult and Juvenile Correctional Health Services, Primary Care Clinic (FQHC), Mobile Medical Clinic, and Pharmacy. Prior to transitioning into his role with the County of Sacramento, he worked for Kaiser Permanente in various managerial roles overseeing specialty clinics such as Dermatology, Mohs Surgery, Surgery Scheduling, Pain Management, and the Memory Center. Prior to joining Kaiser Permanente, Noel spent sixteen years (16) in the public sector as a Child Protective Services Social Worker/Investigator, SSI Advocate, Social Services Supervisor in Employment and Eligibility, and Health Services Manager of the county clinic/call center/mobile medical & dental clinics. Noel has twenty-eight (28) of experience in the non-profit, private, and public sectors and his experience extends to training in the scope of foster care and kinship for various community colleges and UC Davis Extension. Noel currently sits on the Planning Commission for the City of Vacaville, and he is also a member of the Nu Alpha Kappa (NAK) – National Alumni Association (NAA). Noel holds master’s degrees in public administration and organizational Leadership.

Rachel Kay serves as Division Manager for the Sacramento County Health Center with the Department of Health Services, Primary Health Division. She brings more than 15 years of leadership experience spanning healthcare operations, public policy, workforce development, organizational strategy, and marketing & communications. Rachel has extensive expertise leading large multidisciplinary teams, managing complex budgets, and developing collaborative solutions that improve service delivery and strengthen organizational performance.

Prior to joining Sacramento County, Rachel served in senior leadership roles with Huntsman Mental Health Institute at the University of Utah Health, where she directed the statewide integrated crisis response system and suicide prevention programs. She led strategic planning, operational oversight, legislative engagement, and workforce initiatives supporting 24/7 behavioral health services across Utah. Her work included securing public funding, advancing policy development, expanding access to patient care, and implementing innovative programs designed to improve outcomes for individuals experiencing behavioral health crises.

Throughout her career, Rachel has been committed to building high-performing teams, fostering positive workplace cultures, and developing strong partnerships with elected officials, government agencies, healthcare providers, and community organizations. She holds a Master of Business Administration from the University of Utah and bachelors degrees in Management, Marketing, and International Business from the University of Hawaiʻi at Mānoa.

Dr. Gonzalez is a dynamic and innovative leader, with a strong focus on improving patient outcomes and ensuring that all patients receive high-quality care. She brings valuable experience to the Sacramento County Health Center gained through diverse past assignments in intensive care, emergency medicine, hospitalist services, and outpatient care, including more than 6 years of FQHC experience. She has also received several Healthcare leadership certifications which have enhanced her skills managing medical affairs and operations. Dr. Gonzalez has successfully established and scaled new medical departments and rapidly growing primary care clinics by developing and implementing efficient schedules, workflows, and quality care following the most up-to-date medical guidelines. Her strategic initiatives have consistently enhanced productivity, improved patient satisfaction, and achieved top quality metrics.

With a unique passion for pediatrics and prenatal care, she understands the need to prioritize the health and wellbeing of the next generation. Her work on creating new pediatric departments with outstanding growth, exceptional well checks rates, and improvement of early access to care in pregnancy, are some of the many examples of her invaluable vision. This dedication and skill are strong assets for the SCHC team.

As a mentor to medical students and residents, she emphasizes the importance of educating new generations in becoming exceptional healthcare professionals. Furthermore, she prioritizes fostering clinician wellbeing, satisfaction and engagement through effective onboarding processes, ongoing education, and robust support initiatives.

Dr. Gonzalez is fluent in Spanish and has a long history of serving the Spanish-speaking population in healthcare programs throughout United States and Venezuela. This cultural insight is critical to supporting the Health Center’s delivery of services to this population.

Finally, she possesses in-depth knowledge of current medical guidelines, government policies, and insurance regulations, enabling a holistic approach to patient care and operational challenges. Dr. Gonzalez’s commitment to excellence in medical practice and operations drives her ability to deliver high-quality healthcare services, and the Sacramento County Health Center will continue to benefit from this commitment.

Sara Lee serves as the County Pharmacist for the Department of Health Services, Primary Health Division. With nearly three decades of experience, she brings a well-rounded background in both inpatient and outpatient pharmacy, including ambulatory care, psychiatry, hospice, long-term care, and pharmacy management. She earned her Doctor of Pharmacy (Pharm.D.) from the University of the Pacific and completed a residency in geriatrics at the West Los Angeles VA Medical Center.

In her current role, Sara oversees pharmacy services at the County of Sacramento Primary Care Center, the Mental Health Treatment Center, and leads the Clinical Pharmacy Team at the Sacramento County Health Center. She also coordinates and collaborates with the County correctional pharmacies to support shared initiatives.

Sara plays a key role in promoting safe and effective medication-use practices, working in collaboration with clinical and operational teams to help ensure high-quality care. She is dedicated to supporting programs across the department through her work in pharmacy services, clinical support, regulatory compliance, and pharmaceutical budget management. Her broad training and hands-on experience continue to shape her thoughtful, patient-centered approach across the County’s health system

Pamela Gandy-Rosemond holds a Bachelor's and Master's degree in Nursing. She has practiced as a Registered Nurse for over 35 years with specialties in critical care, cardiac, surgical, medical, psychiatric, and correctional health. Her career also includes 22 years of clinical leadership with Sacramento County in the Behavioral Health Division and Primary Health Division. Pamela has been instrumental in developing programs focused on quality and effective patient care for the residents of Sacramento County.

In her current role, she is the Health Services Administrator for Juvenile Correctional Health. She leads a team of medical personnel in transforming the care provided for detained youth to ensure best practice methods and quality care are provided. The facility has been recognized by the National Commission on Correctional Health Care for demonstrating that quality care is provided in accordance with national standards. The facility was recently recertified for an additional 3 years.

Tianna Hammock serves as the Division Manager for the Department of Health Services, Adult Correctional Health, where she oversees and monitors compliance of healthcare services within the Sacramento County jails. In this role, she leads a multidisciplinary team of physicians, nurses, pharmacy, dental, and administrative staff to ensure inmates receive comprehensive, timely and effective healthcare.

Dedicated to advancing quality improvement and compliance, Tianna emphasizes a collaborative approach in meeting the requirements of the Mays Consent Decree. By fostering strong partnerships across clinical staff, custody and administration, she works to align correctional healthcare with community standards, ensuring continuous progress toward safe, equitable, and patient-centered care for Sacramento County’s incarcerated population.
